prefoce The 13th edition of Maternity Nursing reflects extensive revision. Each chapter has been updated, many have been rewritten entirely, and several new chapters have been included. Today, society is changing at a harrowing pace and the institutions within society, particularly the family, are also undergoing extensive reorganization. As the roles within society change, so, too, must those who occupy the roles alter their thinking and behavior to keep pace with the times. Thus, as clients change their orientations and behavior, providers of care must alter their views and behavior to effect synchrony in their mutual roles. Of all the institutions in society, perhaps the family has undergone the most innovations in the last decade. The variety of family forms that appear to be emerging, such as single parent families and communal living, as well as the related issues of family planning and abortion, have, of necessity, broadened the scope of obstetrics and maternity nursing. Moreover, the changes and inno- vations that are occurring within the health care delivery system itself, particu- larly~in nursing, have had an impact on our specialty. Thus, in this edition we have attempted to address these salient issues that affect maternity nursing practice so that students will be aware of their importance and can utilize them as they deliver true family-centered maternity care to their families. Unit I has been reorganized to provide current concepts and thinking regard- ing the changing roles in the family, nursing, and parenting. The authors feel that, in order to help students learn to deliver high-quality family-centered nursing care, it is necessary to give them a broad base of knowledge from which to conceptualize the families with which they work. Thus, the concepts of role, the changing family, professional and parental roles in society, and the social factors in society which place the family at risk during their reproductive years are all introduced and explained to provide this broader conceptual base. Unit II has also been rewritten to reflect the recent advances in knowledge a Oout maternal physiology during reprofluction, conception and ovum develop- ment, and development and physiology of the embryo and fetus, as well as current clinical aspects of human reproduction. This unit supplies the basic physiology, anatomy, and pathophysiology on which students can base their nursing care. Dimensions of effective nursing care in each phase of the reproductive cycle have been enlarged, based on recent research and conceptual developments in nursing practice and related disciplines. The areas of antepartal and postpartal care, parent education, the conduct of normal and complicated labor, care of full-term and high-risk infants, and nursing in emergency situations all reflect this current thought and have been expanded accordingly. Of particular interest is the new chapter on the nursing management of pain and discomfort during labor. Basic theories of pain and the response to pain are examined and specific nursing measures to aid the patient who is experiencing pain and discomfort during labor are described in detail. Current concepts regarding expanded roles for the maternity nurse as well as the role of the nurse-midwife have also been addressed and their implications for maternity nursing practice have been explored. The subject of fetal medicine which was so new at the beginning of the decade has already advanced beyond fetal diagnosis to include current advances
